Dilute lidocaine is injected, and threads are delivered using cannulas and drawn taut to expertly lift the underlying tissues. Additional entry points may be necessary for a more extensive lift. To achieve the best results for your thread lift, various entry points can be considered, including the jaw angle, mastoid region, and areas anterior to the sternocleidomastoid muscle. These areas serve as anchoring points in traditional facelift surgery, making them ideal choices for the PDO thread lift procedure. Once the entry points are determined, the appropriate vectors are carefully evaluated, and the tissues are lifted along the best trajectory to achieve effective and natural outcomes.

Up to three threads can be accommodated per entry point. Before beginning the procedure, the entry point is anesthetized with a local anesthetic to ensure your comfort. An 18-gauge needle is then used to enter the skin, followed by a 21-gauge blunt cannula to dissect the planned vectors in the subcutaneous/supra SMAS plane toward the desired endpoint. To further ensure your comfort, nitrous oxide can be utilized during the process. Once the vectors are in place, the threads are delivered using a cannula technique, with the 19-gauge cannula and thread typically used as the standard. Depending on the tissue's weight, an 18-gauge or 21-gauge cannula may be used. For instance, the temporal point just anterior to the hairline and above a horizontal line from the lateral canthus is usually treated with one thread in the subcutaneous/supra SMAS plane.

This thread travels toward the middle of the nasolabial fold, another toward the middle of the marionette fold, and another toward the bulkiest point of the jowl. The tissues are gently pushed upward, engaging the barbs of the threads, resulting in a lifting effect. Any excess thread projecting from the entry point is then cut after securing additional threads, with up to three threads per entry point. This alone does an excellent job of lifting the midface. For a further lift to the cheek, jowl, submental region, and neck, additional threads may be placed at the mentioned entry points. Remember, the number of threads required depends on the amount of lift needed. Having realistic expectations along with appropriate candidate selection are essential factors for achieving the best results.
